If a patient is dissatisfied with the second provider and wants to change again, his/her request is reviewed and decided upon by designated CCC clinical staff. Often patients are assigned to a particular provider based on the provider’s availability and his/her ability to accept specific insurance plans.
If available, you should bring proof of your HIV-positive status, copies of your health records from previous providers, and your insurance card.
